Getting a $475 refund from Restylane is nothing to sneeze at. To participate, you'll need to submit a copy of your physician's invoice, plus the end flap from the Restylane box(es) that were used in your treatment.

You'll also need to register on the Restylane website  and then fill out and print a Restylane Rewards Redemption Form.

Here's how the cash back program is structured:
  Treatment 1 - $25 cash back
  Treatment 2 - $50 cash back
  Treatment 3 - $50 cash back
  Treatment 4 - $75 cash back
  Treatment 5 - $75 cash back
After your 5th treatment they send you a voucher for $200.
