2- After you click on "get rebate" you will be brought to a rebate form that you can print directly from your computer.
There are 2 parts to the rebate form. One part is for patients who have drug insurance. The other part is for patients without drug insurance.
If you have to pay cash, present the form to the druggist - he will deduct $15.00 off the price of the prescription. The drug company will also pay him an additional $2.50 to do the paperwork.
If you have drug insurance, you are required to fill out the form and mail it in (along with your original receipt) to:
