Explanation: These coupons have been widely distributed to doctors' offices.  There is a good chance that your doctor has a supply of these coupons.

The coupons are part of a comprehensive program designed to increase the chances of a person successfully quitting smoking with Chantix®.

If your doctor does not have a supply of these coupons, he must call his Pfizer representative.  If he does not know who his Pfizer representative is, he should call Pfizer at 1-212-733-2323.  Pfizer will dispatch a pharmaceutical representative to meet with your doctor, drop off the coupons and explain how the program works.

Only doctors who normally prescribe smoking cessation medicines are appropriate to participate in this program (i.e., family practice, internist, endocrinologist, cardiologist, physician assistant, nurse practitioner, etc.) 

Pharmacists may not request these coupons.

This program is generally not appropriate for doctors who do not routinely prescribe smoking cessation medicines (such as surgeons, retired doctors, opthamologists, gynecologists, oncologists, allergists, etc.)

You should know that:

In most cases, you can use the coupon to pay for your copay.  Prescription drug coupons and/or rebates are prohibited in certain states such as Massachusetts.  Check with your local pharmacy to make sure that this offer is valid in your state. 

Drug coupons are not valid for prescriptions for which payment may be made in full or in part under a federal or state healthcare program such as Medicaid or Medicare.

The amount of the rebate or prescription can not exceed the patient's actual out-of-pocket expenses.
